This notice describes how medical information about you may be used and disclosed, and how you can get access to that information.
How we use your health information
We use it to treat you, to bill your insurance, and to run our practice — for example, coordinating your surgery with a hospital or reviewing quality of care. We do not sell your information.
Your rights
- Get an electronic or paper copy of your medical record.
- Ask us to correct information you believe is wrong.
- Request confidential communication at a different address or phone number.
- Ask us to limit what we share, including with your insurer if you pay out of pocket.
- Get a list of who we've shared your information with.
- File a complaint if you feel your rights were violated — we will never retaliate.
